PROBLEM TO BE SOLVED: To provide a forming apparatus of a smart fastener for a clamp which has an automation process including a recessed groove edge forming process, an annealing process, a nut insertion process and a donut type nut support cap forming process.SOLUTION: A forming apparatus of a smart fastener for a clamp includes a recessed groove forming machine 100 which forms a recessed groove edge of a recessed groove shape on an upper surface edge of a head of an eyebolt forming member formed of a tip opening part and a male thread, an annealing furnace 200 which performs annealing of only the recessed groove edge formed through the recessed groove forming machine at a temperature of 300°C to 350°C, a nut insertion machine 300 which inserts a nut to the male thread side of the eyebolt forming member of which the recessed groove edge is annealed in the annealing furnace, and a smart fastener forming machine which folds the recessed groove edge to an inner part direction on the basis of a center point of the recessed groove edge while applying a vertical pressing force of high pressure in the upper edge direction of the annealed recessed groove edge after successively supplying a plurality of eyebolt forming members to which nuts are inserted through the nut insertion machine in a line and, thereby, a donut type nut support cap is formed on a head.
