The device has a derivation unit deriving a hemodynamic index representing tolerance of a patient to spontaneous atrio-ventricular conduction, from an endocardiac acceleration signal delivered by a hemodynamic sensor i.e. endocardiac acceleration sensor, where the signal represents movements produced by cyclic contractions of myocardium. An inhibiting or forcing block (20) inhibits or forces conditional switching of the device towards a dual chamber or biventricular mode based on the hemodynamic index derived by the derivation unit. A switching block (16) switches the device to the mode. The endocardiac acceleration sensor is selected from a group comprising endocavitary or epicardiac acceleration sensor, myocardium wall movement sensor, intracardiac pressure sensor, intracardiac bioimpedance sensor, oxygen saturation sensor and volume variation measurement sensor.
