PROBLEM TO BE SOLVED: To provide a form recognition device, a system, a method and a form recognition program capable of accurately recognizing letters in a form.;SOLUTION: A form recognition device includes: a region determination unit for recognizing a document of a constant format including a form and determining a region regarding line or frame information; a character within region recognition unit for recognizing a character within the region for a specified region within a determined region; an item corresponding to attribute selection unit for selecting an item corresponding to an attribute estimated to be item name information among character information obtained by the character within region recognition means from an attribute-possible item within attribute data base; an adjacent item selection unit for selecting one with high degree of similarity regarding character information other than regarding the information estimated to be the item name information; and a character information confirmation unit for verifying and confirming adequacy of the character information selected by the adjacent item selection means. Furthermore, a plurality of formula expression patterns are associated with a same form pattern.
