Embodiments of the invention include a system for helping a patient adhere to a care plan, the care plan designed to be followed by the patient in order to improve the patient's health, the system comprising: a task scheduling module configured to schedule one or more tasks included in the care plan, wherein a scheduled task is intended to be completed by the patient at the scheduled time; a task adherence estimation module configured to estimate the likelihood that the scheduled task will be completed by the patient at the scheduled time; and an alternative task presentation module configured to present the patient an alternative task, wherein the alternative task is intended to replace the scheduled task when the task adherence estimation module determines the patient is unlikely to complete the scheduled task and when the alternative task is accepted by the patient.
