PROBLEM TO BE SOLVED: To provide a dosimeter which makes it possible to simply measure a radiation dose of a patient surface and is also easy to handle after that.SOLUTION: A sheet-like dosimeter comprises a radiation-sensitive gel composition comprising a monomer polymerizable by radiation exposure and a gel matrix having the monomers dispersed therein, and a transparent package having the gel composition sealed therein, wherein the package has an oxygen barrier property at least during radiation exposure.
