Disclosed are systems and methods for evaluating patients with vascular disease. One method receives one or more vascular models associated with either the patient or multiple persons, receives observation perfusion information associated with the patient, and further uses one or more computer processors. Estimating one or more blood flow characteristics or one or more pathological features of the patient based on the observed perfusion information and the one or more vascular models.
