PROBLEM TO BE SOLVED: To provide a form and an electronic manifest information registration system capable of easily utilizing a waste management system by an electronic manifest even if a waste disposal agent has no know-how on the electronic manifest.;SOLUTION: A form used for an electronic manifest information registration system, which is a form to be entered by an industrial waste disposal agent, provides at least an entry column for information specifying the waste disposal agent, an entry column for collection date of industrial waste, and an entry column for a collection amount of industrial waste. The electronic manifest information registration system comprises: the form; a terminal device used for a collection/transportation agent that collects the industrial waste from the waste disposal agent and transports the same, and that creates waste collection data that collects input information by a spreadsheet function when information entered in the form is input; and an information management server, which is connected to the terminal device via a communication network, receives the waste collection data from the terminal device, converts it into the CSV format, and transmits it to an EDI server on the agent side.;SELECTED DRAWING: Figure 1;COPYRIGHT: (C)2018,JPO&INPIT
