A conjugated polymer nanometer probe comprising: a fluorescent conjugated polymer; a surface ligand; a target molecule; and a near-infrared fluorescent dye; and may also comprise magnetic resonance contrast agents with gadolinium. The preparation method for the conjugated polymer nanometer probe involves: adding the above mentioned materials in an organic solvent, ultrasonic mixing to obtain a mixed solution; and adding the mixed solution in ultra pure water, continuing the ultrasound. The conjugated polymer nanometer probe merges near infrared fluorescence imaging, photoacoustic imaging and magnetic resonance imaging bi-modal or tri-modal molecule imaging technology, can recognize metastatic lymph nodes and normal lymph nodes, and where the metastatic lymph nodes stay for a long time period, satisfy the requirements for long-term observation. At the same time, the near infrared fluorescent conjugated polymer nanometer probe can generate active oxygen under light so that it can be used for tumor photodynamic treatment.
