The present invention relates to a medical image processing system and method for personalized brain disease diagnosis and state determination, the system and method being capable of predicting a current brain state, a disease-specific risk degree, and a disease by obtaining an image through a noninvasive magnetic resonance image and analyzing composite images of brain tissue and a cerebrovascular part, and the system comprises: an image processing unit for obtaining a three-dimensional TI weighted image, a two-dimensional T2 fluid attenuated inversion recovery (FLAIR) image, a magnetic resonance angiogram (MRA) image for imaging only the blood vessels so as to confirm abnormality in the cerebrovascular part, and a four-dimensional phase-contrast flow image for identifying a state of blood flowing in the blood vessels; a complex image analysis unit for selecting a disease to be diagnosed and analyzing brain tissue and vessels by setting a brain area according to the disease; and a personalized diagnosis and result output unit for outputting a brain state, a disease-specific risk degree, a risk of disease, and a disease prediction result through a machine learning algorithm, by utilizing an age-specific data DB.
