To provide a prediction method of low nutrition and frailness for a general elderly person, and to provide a tool, a device, and a program capable of being used for prediction.SOLUTION: The invention is a method including the steps of: scoring replies to question items relating to, for elderly person, nutrition related recognition, an eating state, a body activity condition, and eating-related quality of life (QOL); and determining a risk that the elderly person becomes low nutrition or frailness on the basis of a total score.SELECTED DRAWING: None
