The present invention provides a system for communicating a referral document from a first health care service provider to a second healthcare service provider, the system comprising : a first computer of a health care referrer, the first computer having a database comprising information on a plurality of patients, the first computer configured to generate and transmit patient-specific referral information, a second computer of a health care referral recipient, the second computer having a database comprising information on a plurality of patients, and a third computer of a coordinating entity in operable communication with the first and second computers, the server configured to receive and store the patient-specific referral information transmitted by the first computer and permit importation of the patient-specific referral information into the database of the second computer.
