A healthcare system that facilitates patient-customized healthcare services from multiple healthcare organizations via a single healthcare application is disclosed. A mobile computing device authenticates a patient with a first healthcare server application associated with a first healthcare organization. The mobile computing device receives from the first healthcare server application a plurality of healthcare organization (HCO) pages, at least some of the plurality of HCO pages comprising information specific to a first medical condition of the patient. The mobile computing device generates a user interface based on a plurality of common user interface components maintained in the mobile computing device and the first plurality of HCO pages, and presents, on a display device of the mobile computing device, the first user interface, the first user interface including the plurality of common user interface components and at least some of the information specific to the first medical condition of the patient.
