Embodiments described herein provide a method and apparatus for creating a representative medical report customized for a specific patient. The method retrieves medical findings for the patient and allows a doctor or user to enter those findings into a virtual realization screen. The virtual realization screen allows the doctor to pin localized conditions to the locations on the specific patient where the condition occurs. Diffuse conditions are painted onto affected segments. Multiple segments of a patient's body or body system may be jointed together so that the patient is teleported through their specific medical report.
