The present invention provides a composition for preventing, alleviating, reducing, or eliminating post-inflammatory hyperpigmentation, comprising, as active ingredients, at least one selected from the group consisting of high-purity exosomes and/or extracellular vesicles derived from stem cells and having a uniform particle size distribution, and fractions containing high-purity exosomes and/or extracellular vesicles isolated from a stem cell culture medium and having a uniform particle size distribution. The composition of the present invention can inhibit the production of TNF-α, IL-6, IL-1β, and nitric oxide (NO) that stimulate melanocytes, and thus can ultimately reduce melanin production, and furthermore, can penetrate deeply into the dermal layer where melanin pigments are deposited, and thus can effectively prevent, alleviate, reduce, or eliminate post-inflammatory hyperpigmentation. In addition, the composition of the present invention, when applied to the skin using iontophoresis, laser beam irradiation, IPL irradiation, and/or RF energy application, allows exosomes and/or extracellular vesicles to penetrate deeply into the dermal layer of the melanin-deposited skin, thereby maximizing the cosmetic effect or therapeutic effect for the skin, and in particular, the composition of the present invention can cause synergistic effects in reducing or eliminating pigments by laser beam irradiation, IPL irradiation, and/or RF energy application.
