The present invention relates to a composition for preventing, alleviating, or treating hepatic fibrosis, comprising exosomes derived from adipose stem cells as an effective component. The exosomes derived from adipose stem cells according to the present invention are loaded with genetic information, proteins, growth factors, etc. for treatment of hepatic fibrosis, and thus showed excellent therapeutic effects on hepatic fibrosis in an animal model having hepatic fibrosis, and since the exosomes are material derived from cells, the exosomes have advantages including good biocompatibility as well as excellent absorption rate. Accordingly, the present invention can minimize the limitations of therapeutic drugs currently in use, stem cells turning into cancer cells that may occur during the treatment using stem cells, or adverse side effects caused by cellular debris. Also, since the composition of the present invention can be naturally delivered to the liver via intravenous injection, topical administration, or the like that can be simply applied without surgery, and thus can reduce the costs and the burden on a patient. Consequently, the exosomes derived from adipose stem cells according to the present invention can be beneficially used for preventing, alleviating, or treating hepatic fibrosis.
