METHOD FOR TWO-STAGE SURGICAL MANAGEMENT OF COMPLICATED PATHOLOGICAL FRACTURE (DISLOCATION) OF PHALANX OF FINGER ON BACKGROUND OF BONE (BONE-ARTICULAR) WHITLOW

FIELD: medicine.;SUBSTANCE: invention refers to medicine, namely to traumatology and surgery. Treatment is performed in two stages. At the first stage, whitlow incision is performed, then necrosectomy, sequestral necrectomy, excising of necrotic soft tissues and bone sequesters are performed. After excising the necrotic soft tissues and bone sequestering of the finger segment on the open wound, conservative treatment is performed until the granulations are formed and purified. That is followed by a second stage of intramedullary introduction of the spicules, whereby osteosynthesis of finger phalanx fragments and/or arthrodesis of the distal interphalangeal joint is performed. That is followed by plasticity of the formed soft-tissue defect with a non-free skin-fat graft from a neighbouring finger on a wide base, after which the donor flap is cut off from the feeding base and the formed wound is closed on the donor finger.;EFFECT: method enables to preserve a finger segment by closing a soft-tissue defecate, to flesh the area of a pathological fracture, to eliminate a phalanx dislocation, to achieve good cosmetic and functional results.;1 cl, 1 ex, 5 dwg
