An alveolar mucosa is incised within a defect within healthy tissues, a trapezoidal mucoperiosteal flap for wound closure is formed, resection of alveolar process is performed by single bone block within one segment with preservation of continuity of bone structure of jaw, bone cavity is filled with thrombocyte enriched blood auto clot, along periphery of bone cavity is provided with compactosteatomy, wherein for complete separation of bone cavity with oral cavity periosteum is dissected in base of formed flap and two incisions are made as poker to increase its length and free movement into zone of operative field and close defect, and in case of complete wound closure, additionally, a portion of the Bichat’s fat pad is used in the upper jaw osteonecrosis or a tongue-shaped fat-plastmas flap in lower-jaw osteonecrosis, which are moved into the postoperative defect for two-layer separation of the wound with the oral cavity.;EFFECT: method allows improving the blood supply in the defect area, ensuring effective dissection of the bone cavity in the area of the defect with the oral cavity, reducing postoperative complications, reducing the wound healing time.;1 cl, 4 dwg, 2 ex
