A case of rhynogenic optic neuropathy is herein reported. The case was a 67-year-old female who had undergone the operation for chronic sinusitis 50 years ago. She complained of nausea and vomiting, and was admitted to the department of neurology suspected of meningitis. After admission, she developed visual disturbance, and was referred to our department from that of ophthalmology. The CT scan showed that mucocele of the right posterior ethmoid and sphenoid sinuses compressed the optic nerve. Based on the diagnostic findings, fiberscopic intranasal surgery was performed without delay to prevent permanent visual impairments. Visual acuity and visual field recovered 5 days after surgery.
