Home Visiting: The Gateway to Early Intervention for Inner City Minority211 Populations

The purpose of the Gateway grant to Little Sisters of the Assumption Family211u001eHealth Service was to enhance LSAFHS's capacity to reach, assess, address and 211u001emonitor the needs of high risk pregnant and postpartum women and their newborns 211u001ein East Harlem; to provide preventive interventions and developmental tracking 211u001eservices to infants and toddlers at-risk for developmental delays, and; to fully 211u001eparticipate in the New York State Department of Health Title V, Part H Early 211u001eIntervention Program (EIP) as qualified early intervention evaluators and 211u001eproviders. The model is grounded in the realization that vulnerable newborns born 211u001eto families at risk because of existing and emerging risk profiles require a more 211u001edynamic and continuous assessment of risk, which may shift from emphasizing 211u001ematernal risk to fetal and infant risk according to the particular stage and 211u001econfiguration of risk factors.
