Citizen Action for Community-Based Care: The Wisconsin Model. Long-Term SupportPlanning Committee Handbook. Change Through Action: A Model Training Package for Empowering Consumers in the Aging and Disability Communities

摘要

The coalition works closely with the formal aging network to advocate for changesin public policy and is the state-designated agency for the administration of the federal protection and avocacy programs for people with severe disabilities and mental illness. Under this grant, these two organizations (CWAG and WCA) jointed forces to empower older people and people with disabilities to become more involved in all aspects of home and community-based care from advocacy for changes in state policy to being more directly involved in the design and delivery of their own case plans. The goals are: (1) to increase the consumer participation in c-b care. (2) to revitalize Wisconsin's formal structure for requiring consumer participation through the county long-term support planning committees; (3) to establish three model cross-disability coalitions mobilized around expanding home and community-based care programs.
