Since the nature and extent of mental illness and addictive disorders can change over time, the authors emphasize data collected in the 1990s. The authors include older data where they might be useful and if they are the only available data on the topic. The data are the most recent that the authors were able to obtain as of publication preparation date. The authors present trends when they are available.
