Classification procedures and forms related to the work of human relations agencies are included in this document, and are part of a system developed by the Charlotte - Mecklenburg Community Relations Committee in North Carolina to classify and describe the work of local government in the area of human relations. An analysis of materials from and interviews with human relations agencies in North Carolina indicated that the three general classifications of human relations work are problems, clients, and processes used to solve problems. Processes are defined as types of actions taken by human relations agencies to solve problems, results of these actions, and measurements used to evaluate actions performed. Procedures and forms are designed to keep records of all types of information encompassed by the classifications of human relations work. To use the forms, each member of a human relations agency staff should make an original record of all work performed using either a case report form or an activity log form and also summarize activities monthly according to process. Nine activity report forms have been designed to accomplish the latter.
