Report to Congress on Abnormal Occurences, April-June 1987

During the report period, there were no abnormal occurrences at the nuclear power plants licensed to operate. There were five abnormal occurrences at the other NRC licensees. Three involved medical misadministrations (two diagnostic and one therapeutic); one involved the issuance of an NRC Order to remove a hospital's radiation safety officer due to falsification of certain records; and one involved a significant breakdown in management and procedural controls at an industrial radiography licensee. There was one abnormal occurrence reported by an Agreement State (Idaho). The item involved radiographer overexposures. The report also contains information updating some previously reported abnormal occurrences.
