A 12 years old Thoroughbred gelding stepped onto his clip and suffered an infected pedal bone fracture without joint participation. The isolated bone fragment could not heal because of a superveining osteomyelitis, and the infection could not be contained. First the surgical extraction of the infected sequestrum brought the infection to a standstill and the wound proceeded to heal. The hoof mechanism was eliminated with a cast. The horse received systematic and local antibiotics along with NSAIDs. Two weeks after the surgical extraction the horse was discharged. An orthopedic shoeing was fitted and after three months of stall rest the horse slowly started training again. Seven months post surgery, the horse was in full driving training, although five minutes warm up were and are still necessary before the horse was sound.
