A 21-month-old boy with fever and red eye.

A 21-month-old Hispanic male patient presented to the emergency department with fever and right eye redness. On the day of presentation, he awoke with unilateral right conjunctival injection and a white "patch" over the bottom portion of his right iris. There was no eye discharge. His mother had not noticed any changes in his gait or any indication of visual disturbance. She did not recall eye trauma. She complained he was more sleepy than usual and reported 3 days of tactile fever and nonbloody, nonbilious vomiting. He had a normal number of wet diapers that day. There were no ill contacts. He did not attend day care.Two days before presentation, he was seen for a rash in an outpatient clinic and diagnosed with roseola.He had a laryngitis-type illness 1 week before this illness began. It resolved without medication.There was a history of travel to Mexico for 4 days 5 months previously. In Mexico, there were no known exposures to tuberculosis or other illnesses.This patient was born at term without anyaccompanying complications. He had not had any hospitalizations or. surgeries. He was not taking any medication. He had no allergies to any medication. His immunizations were up-to-date.
