Transition is defined as the movement from adolescence to adulthood in all areas, including home, health care, education, and community. Transition should be a process rather than an event. Optimally, a child, his or her family, and the practitioner should be preparing for transition throughout childhood and adolescence. Although all adolescents make this transition, this article focuses on the transition process for vulnerable populations, including those who have chronic medical conditions, intellectual disabilities, and mental health conditions.
