DS Smith Paper Ltd was fined after an accident in which a worker was crushed between two trucks at their Bolton recycling plant, suffering fractured bones, a punctured rib and multiple bruising. The HSE investigation found the firm failed to observe correct safety procedures around the tipping area. Since the incident the company has introduced new safety procedures. Northern Tissue Group (Papermill) Ltd has been fined following an incident in which a mezzanine floor, 6m above the ground, gave way. The HSE investigation found the company did not have a suitable inspection and maintenance regime for checking the floor, despite it being in a humid area and at risk of rusting.
