Aims. - The aim of biomechanical foot orthotics is to compensate a postural and dynamic disorder leading to an iliotibial band syndrome in one cyclist. The purpose of motion analysis is to quantify the effect of foot orthotics in this cyclist. Method. - One trained cyclist (age, 31 years; body mass, 72 kg; height, 178 cm) has participated in this study. He suffers from an iliotibial band syndrome in left knee and he has a postural and dynamic disorder with varus feet and genu varum. Two sets of tests are conducted, the first without orthotics and the second with orthotics. The cyclist's movements are recorded by a 3D motion analysis system (Motion Analysis Corporation, Santa Rosa, CA, USA). Kinematic joint angles of ankle, knee and hip and distance between knee center and bike are studied. Results. - The results show that the orthosis reduces the distance between knee center and bike (p < 0.001) through joint coupling starting at foot and affecting upper joints. This coupling creates an internal rotation of thigh (p < 0.001) and shank (p < 0.05), knee valgus (p < 0.001) and hip adduction (p < 0.001).
