Leaking 'outside the box': A unique o-ring failure mode

As a product design engineer in the rubber seal industry for more years than I care to admit, I've personally analyzed thousands of failed o-rings removed from leaking customer applications. When a unique new failure mode appears, it deserves a little extra investigation. In this case, a combination of factors combined to create a new mechanism of o-ring failure.
