The 1999 US Institute of Medicine Report 'To Err is Human' stimulated immediate headlines and global dialogue about the nature of medical error and the dilemma of how to improve patient safety (Institute of Medicine, 1999). The world quickly grasped the significance of the estimate that between 44 000 and 98 000 people per year die because of medical error in the USA. Page 1 of the report characterized the urgency of the issue in unforgettable terms:'Even when using the lower estimate, deaths due to medical errors exceed the number attributable to the 8th leading cause of death. More people die in a given year as a result of medical errors than from motor vehicle accidents (43,458), breast cancer (42,297) or AIDS (16,516).' (US Institute of Medicine, 1999).
