The evolution of mechanical and pharmacologic therapies for percutaneous coronary interventions (PCI) has been so fast paced in recent years that questions regarding what is "optimal" clinical practice have emerged quicker than they can easily be answered. During this same interval the rate of adverse events associated with PCI has progressively decreased, and as a result the corresponding sample size to discern a meaningful difference between current practice and new treatment strategies has reciprocally increased. Randomized clinical trials therefore often need to be quite large and have a lengthy enrollment period. These challenges have led to heightened "off label" product usage and a tendency to turn to registry data if they can be made quickly available and are of high quality.
