Equipment entrapment in the coronary tree can occur under a variety of circumstances. Primary manufacturing defects are extremely rare and the vast majority of events usually arise when devices are used beyond the control of the manufacturer. A classic source of misadventures is the heavily calcified artery that either entraps the balloon material or causes the operator to exceed bursting pressures resulting in unexpected outcomes. Applying excessive tor-k que especially if there is anchoring of the distal wire that tethers the device is a set up for wire fracture and similar device misadventure. Failure of a balloon to deflate is a very rare situation. This can occur if either there is kinking or closure of the lumen supplying the fluid for balloon inflation or plugging of the lumen with solid material such as foreign bodies or crystallized contrast.
