In the early stages of the disorders, the clinical symptoms of progressive supranuclear palsy (PSP) and of idiopathic Parkinson syndrome (IPS) may be very similar. Dysarthria frequently occurs in both disorders. In the present article we discuss a potential role of dysarthria in the differential diagnosis of PSP and IPS. Sixteen patients (nine with PSP, seven with IPS) were examined with the Bogenhausen dysarthria scales (BoDyS), the Munich Intelligibility Profile (MVP-Online), and a self report questionnaire. Furthermore, acoustic analysis methods were used to measure speech rate in sentence repetition. The majority of PSP-patients had moderate to severe dysarthric impairment and decreased intelligibility at rather early stages of their disease, whereas the IPS patients examined here had relatively mild dysarthria and good intelligibility, even after long histories of Parkinson's disease. Regarding speaking rate, two patterns could be discerned in the PSP group: patients with markedly slow rates and a comparatively preserved intelligibility, on the one hand, and patients whose speech rates were normal, despite a considerably impaired intelligibility, on the other.
