Venous leg ulcers in obese patients

abstract_textpThe increasing percentage of obese patients in our population is a new challenge for the phlebologic surgeon. Especially for obese patients who suffer from venous leg ulcers, a special infrastructure of the clinical practice and hospital is necessary. The basis of any diagnostics in the field of phlebology is duplex investigation to precisely show the venous anatomy and haemodynamics. Compression therapy in obese patients is very often poorly accepted because of their calf shape or restricted body movement. Beside oedema reduction, treatment of the venous reflux is the basic therapy for healing. This may be done with classical surgery or endovenous methods. Afterwards, the ulcer can be selectively treated with shaving, fasciectomy, and skin transplantation. Patients for whom surgery is contraindicated can be treated by duplex-guided foam sclerotherapy./p/abstract_text
