Muntener and colleagues have evaluated the immediate effect of systemic nitric oxide (NO) augmentation on bladder outlet resistance. Ten, young, symptom-free men had performed two successive pressure flow studies, the first without and the second after a sublingual isosorbidedinitrate. A small but statistically significant mean arterial pressure drop after medication was observed as the systemic effect of the NO donor. Urodynamic (pressure flow) analysis to determine outlet properties was done on the basis of average flow, average detrusor pressure, and detrusor pressure at maximum flow. I have some concerns about the generaliz ability of these observations, and I have even much more concern about the extrapolation of these data to men with lower urinary tract symptoms, benign prostatic enlargement, benign prostatic obstruction, and or bladder outlet obstruction by the authors and in the editorial comments.
