Morgan et al reported the ethnic comparison (similarity and discrepancy) on risk factors of coronary heart disease or stroke using Markov style computer model [1]. They assumed that cardiovascular disease attributable to increase in body mass index (BMI) was mediated through metabolic components such as systolic blood pressure, dyslipidemia and diabetes based on an analysis of Framing-ham Heart study. We agree that the validity of their method on risk assessment is mathematically kept. But there is a fundamental concept that visceral obesity is also one of the metabolic components, and parallel handling of these risk factors is well distributed [2]. Kurth et al have reported that there was a positive relationship between obesity and stroke stratified by gender and type of stroke [3,4]. In addition to mortality study, risk assessment of incidence of stroke have been conducted [5,6], and there was a discrepancy of results on gender difference in the occurrence of stroke.
