In February 2015, the Department of Health (DH) in England launched a series of plans to enable people, including staff members, to raise and escalate concerns about the health service without fear of recrimination. Entitled Culture change in the NHS (DH, 2015a), this initiative is designed to illuminate the actions that have been implemented since the Francis inquiry into the events at the Mid Staffordshire NHS foundation Trust.
