A 70-year-old woman presented with increasing shortness of breath. A chest radiograph showed curvilinear calcification projected over the cardiac silhouette (Figure l(a) and (b)). A helical contrast-enhanced mul-tidetector computed tomography angiogram of the thorax demonstrated extensive pericardial calcification sparing the apex of the left and right ventricles. (Figure l(c)). A sagittal reconstruction of the computed tomography revealed extensive pericardial calcification surrounding the main pulmonary artery and both atria. (Figure l(d)). The patient underwent successful pericar-diectomy through a median sternotomy; the whole pericardium except for the apex of the left ventricle was heavily calcified.
