Inspection and risk assessment failures end in tailgate crush

A Veolia company has been fined ￡750,000 after a worker was fatally crushed by the tailgate of a refuse collection vehicle (RCV) that was being refurbished. The HSE's investigation found that Veolia ES Sheffield (Veolia), which collects and recycles waste, had failed to include a faulty safety critical switch in its inspection regime for the RCV. The company that carried out the work, John Fowler and Son (Blacksmiths and Welders) (JFS), had failed to carry out an adequate risk assessment or draw up a method statement that addressed the risk of the tailgate closing inadvertently.
