In this issue of Blood, Bateman and colleagues identify the ETV6-RUNX1 fusion as a common prenatal event at the origin of t(12;21)(p13;q22) ALL. Additional genetic lesions in the preleukemic clone are required but follow divergent evolutionary pathways toward overt leukemia.
