A 34-year-old man with progressive behavioral and language disturbance.

Case presentation. A 34-year-old man was referred to the Mayo Clinic for evaluation and treatment of behavioral problems and language disturbance. The patient's history was provided by his parents and notes that his wife had prepared for the review, as the patient was noncomnmnicative. Fourteen months prior to presentation, he developed lower thoracic dermatomal varicella zoster (shingles). The rash cleared, but the patient thereafter underwent a progressive change in his personality and behavior. His wife felt that he had become inappropriately mean-spirited and would refuse to participate in the care of his children. He would eat their food and knock their games over. The patient once left his young daughter unsupervised at home. He became argumentative and inclined to spend large sums of money. He became more religious and began purchasing many religious rock CDs. He was selfish and neglected his personal hygiene. He would walk out of the bathroom entirely undressed. He would cram food in his mouth without stopping to chew. He would wander and pace, touching and aligning objects, and he became preoccupied with checking locks. If a box of facial tissues was placed in front of him, he would take these out one by one until they were all gone.
