Preparation of granulocyte concentrates by apheresis: situation in the Netherlands.

Granulocyte transfusions have been used for many years in neutropenic patients with life-threatening infections. Over the years, the granulocyte component was prepared during a time-consuming procedure using a substantial number of whole blood donations. The introduction of the administration of granulocyte colony-stimulating factors (G-CSF) in combination with steroids enables to acquire sufficient amounts of granulocytes in stimulated donors. Moreover, these collected granulocytes are more functional and are also shown to be more resistant against apoptosis [1,2]. These granulocyte components are collected with leukapheresis methods. Recently, the Vox Sanguinis International Forum demonstrated a one centre to the next variety in the collection of the granulocyte components [3]. With this letter, we would like to give our experience in the collection of granulocytes. The answers to the questions are based on the guidelines for the preparation and use of granulocyte components as practiced in the Netherlands national blood transfusion organization Sanquin Blood Foundation. Agreements were made with paediatric departments of university hospitals.
