Salpingo-oophorectomy at the Time of Benign Hysterectomy: A Systematic Review

There is a clear survival benefit for prophylactic oophorectomy in patients with inherently higher cancer risk such as those with BRCA1 and BRCA2 and other genetic mutations. For average-risk women, however, cancer risk reduction must be balanced with the consequences of loss of ovarian hormone production and fertility. The incidence of bilateral salpingooophorectomy (BSO) at the time of hysterectomy declined from 55% in 1999 to 35% in 2011. This decrease was likely due to published studies evaluating large numbers of women and recommendations from the American College of Obstetricians and Gynecologists as well as other professional organizations, suggesting that for women at average risk for ovarian cancer, there are health benefits for ovarian conservation at the time of hysterectomy.
