Diagnosis of Heterotaxy Syndrome in a Patient With Multiple Congenital Cardiac Malformations Using Magnetic Resonance Imaging

A 22-year-old female patient admitted to the emergency department with symptoms of long-standing dyspnea and newly onset palpitation. On ECG, she had atrial fibrillation with rapid ventricular rate. Transthoracic echocardiogram showed dextrocardia with enlarged right heart cavities and left atrium. She had severe tricuspid regurgitation (Movie I in the Data Supplement) with systolic pulmonary artery pressure of 87.25 mmHg (Figure 1). Subaortic type ventricular septal defect was also present (Figure 2A and 2B; Movies II and III in the Data Supplement). We decided to perform cardiac magnetic resonance imaging to reveal additional cardiac or vascular abnormalities. Magnetic resonance imaging showed dextrocardia, midline liver, and polysplenia (Figure 3A and 3B).
