In the following, we present a case with a rare and potential life-threatening complication to vitamin-K antagonists (VKA) treatment. A 49-year-old man presented symptoms of throat pain, difficulty of speech, and a subjective feeling of swelling of the tongue and the floor of the mouth. He had a history of hypertension and was treated with warfarin after the insertion of a prosthetic aortic valve in 2008.Flexible fibre-optic laryngoscopy showed oedema of the tongue base extending to the left side of the epiglottis that was displaced approximately 1 cm posterior towards the hypopharyngeal wall. The suspicion of haematoma was confirmed by an ultrasound scan, and laboratory tests showed an INK. of > 10.
