Pleural disease management has in the past decade evolved from a primarily eminence-based space to an evidence-based one, owing to the unparalleled development of a robust, multicenter, and international research infrastructure, which has resulted in the publication of high-quality comparative clinical studies. From the optimal management of recurrent malignant pleural effusions to a paradigm shift in the way we approach pleural space infections, the breadth of quality data published in the past 10 years by a handful of expert coordinating centers across the globe has been a truly admirable and fundamentally informed clinical practice.1"3 The urgent need for the establishment of similar research networks by the interventional pulmonology community has lately been highlighted by the recent and unprecedented introduction of technologically advanced diagnostic and therapeutic platforms, often with limited or no evidence to support their use in clinical practice. Approval of these novel technologies has almost exclusively been obtained through a US Food and Drug Administration 510(k) clearance pathway that appears increasingly ill-suited to satisfy the necessarily stringent safeguards of safety and efficacy allowing optimal, patient-centered clinical decisions.4
