When the US Congress approves universal health coverage, tens of millions of new patients will be added into an already overburdened health care system. Compounding the demand on our health system is the growing population of people over 65 years of age who have complex chronic care needs. Growing evidence demonstrates that our nation's leading diseases-ie, cardiovascular disease, diabetes, and obesity-can largely be reduced or avoided through education that fosters changes in patient behaviors. How will we provide care for the growing number of individuals?
