To the Editor I read this article with interest. It is reassuring that insurers appreciate our work and accept modifier 22 for the appropriately selected procedures that required greater than usual effort and complexity. Missing, however, was a list of the specific indications for choosing this modifier. Was it chosen based upon preoperative conditions, intraoperative findings or other factors? Without this information, the reader will not know how to apply these results to their own practice, with the potential risks of overuse and underuse. We all can agree that surgeons should receive reimbursement consistent with complexity and effort, notwithstanding practice type, as many of these complex procedures are also being performed appropriately outside of tertiary care centers by subspecialists. We all as a health care community should be justly reimbursed for providing this type of care.
