Who among us has not been frustrated by a patient similar to the ones presented in this article? The authors describe four patients with non-arteritic anterior ischaemic optic neuropathy (NAION) risk factors who present with asymptomatic disc swelling. The patients eventually progressed to classical NAION (hyperae-mic disc swelling and visual field loss) over a period of 2 to 10 weeks. Although the title of the article includes "asymptomatic", it is interesting to note that three of these patients described photopsias prior to visual field loss. The authors highlight the fact that we have nothing in our armamentarium to prevent these patients from suffering vision loss.
