Based on extensive clinical experiences characterized by patients' attempts to prevent the examination of body parts experienced as the cause of chronic pain, it was speculated about which dynamics could be behind this conflict. It has been postulated that early pain, if not moderated by the mother, must be linked to this very same maternal introjection and must be maintained as the only remaining organizing core to prevent fragmentation. Morgenthaler's model of a filling formation in perversions was used as a theoretical framework. Generalizing, the coupling of instinctual acts and affect is referred to, according to which an instinctual action inappropriate to the affect serves for the sedation of precisely this affect.
